What info is put on military dog tags?

I’m in Army ROTC and it takes FOREVER to get anything issued to us because we are bottom of priority list (Not that I’m complaining, I know the best should go to those who need it!). Anyway I want to get some made so does anyone know the info on the dog tags and a link to where I can get them made?
