That he is a lunatic and has mental issues and should be evacuated from the battlefield for evaluation and treatment. Also it means he has committed a war crime.

It means his dog tags can listen for any signs of danger.

usually it means that he killed an enemy soldier and took his ear as a trophy. same with fingers or teeth.
